Health Screening & Education Center

Screen. Understand. Connect. Follow Through.

AIFS helps immigrant and refugee families and other community members make sense of preventive health information, navigate possible next steps, and stay connected to care. The Center is a prevention, education, navigation, referral, and continuity-of-care model—not a diagnostic clinic.

About the Center

Prevention and navigation shaped around real community barriers.

AIFS uses culturally and linguistically responsive service to make health information easier to understand and act on.

The AIFS Health Screening & Education Center supports African immigrant and refugee families, other immigrant and refugee community members, and other community members facing barriers. The Center offers a place to ask questions, take part in confirmed screening or education activities, and understand possible next steps.

AIFS is not a diagnostic clinic. The Center does not replace a licensed healthcare provider for diagnosis, treatment, or ongoing medical care. Its role is to screen and educate, help people navigate, make appropriate connections, and support continuity when that support is part of the confirmed program.

Services

Screening is the starting point—not the whole care journey.

The Center brings together prevention, understanding, navigation, referral, and follow-through. Specific services can vary by event, staffing, and healthcare partner participation.

01

Screening activities

Preventive health screening activities and conversations offered through the center, community events, or partner locations. The specific screening offered depends on the confirmed event and service scope.

02

Health education

Plain-language information that helps people understand prevention, screening information, and questions to bring to a healthcare professional.

03

Community Health Worker navigation

Culturally responsive support to understand possible next steps, navigate systems, and address practical barriers. The support available depends on the confirmed program.

04

Referral and continuity support

When appropriate and available, AIFS can help connect a person with a healthcare or community resource through a warm referral and support follow-through. Partner participation and follow-up scope vary.

Important: A screening may identify information to discuss, but it does not establish a diagnosis or prescribe treatment.

How It Works

Four plain-language moments from screening to care.

Your visit may look different depending on the event or program. AIFS can confirm what is available when you schedule or call.

01

Screen

Start with the screening or health-education activity confirmed for that event. Screening information is not a medical diagnosis.

02

Understand

AIFS helps explain the information in clear, culturally and linguistically responsive language so you can ask questions.

03

Connect to Care

If another conversation or service may be appropriate, AIFS can explain possible referral pathways and next steps.

04

Follow Through

Where the confirmed program includes navigation follow-up, Community Health Workers can help address barriers and support the next connection.

RELAIS Model

From Screening to Care — The AIFS RELAIS Pathway

RELAIS gives the Center a shared way to move from reaching people to helping them stay connected. It is a navigation and continuity framework, not a clinical diagnosis protocol.

01

Reach

Bring prevention information and screening closer to families through the Center, community events, and partner locations throughout the Omaha metropolitan area.

02

Evaluate

Listen to the person’s questions and understand the screening or navigation need. This step does not replace clinical evaluation or diagnosis.

03

Look for Risk

Notice screening information or barriers that may need attention, then explain what is known and what still needs confirmation.

04

Advise

Share practical, understandable information and help the person identify questions and possible next steps.

05

Integrate

Connect the person with appropriate care or community resources when a referral pathway is available, using a warm handoff where the confirmed program supports it.

06

Sustain

Support continuity and follow-through when the confirmed program includes this service. The exact follow-up process depends on staffing, consent, event scope, and participating partners.

Reach • Evaluate • Look for Risk • Advise • Integrate • Sustain. The exact screening scope, referral destination, and follow-up method are confirmed by program, event, staffing, consent, and healthcare partner participation.

Referrals & Follow-Up

A connection is more useful when the next step is clear.

AIFS helps people understand what may come next without presenting the Center as the provider of diagnosis or treatment.

01

Warm referrals

When a confirmed pathway is available, AIFS can help connect a participant to an appropriate healthcare or community resource and explain the handoff. Specific provider relationships are not listed until confirmed.

02

Barrier resolution

Community Health Workers can help identify practical questions about language, forms, appointments, transportation, or system navigation. The specific support available depends on the program.

03

Closed-loop follow-up, where supported

RELAIS ends with Sustain: helping a person stay oriented to the next step when the confirmed program includes follow-up. AIFS does not promise a particular follow-up timeline, outcome, provider, or service outside the confirmed program scope.
