About the Center

Welcome

The AIFS Health Screening & Education Center supports African immigrant and refugee families, other immigrant and refugee community members, and other community members facing barriers to care.

We offer a place to ask questions, take part in preventive screening and health education, and understand your next steps, in a language and a way that makes sense to you.

Please note: AIFS is not a diagnostic clinic. We screen, educate, help you navigate, and connect you to licensed healthcare providers for diagnosis and treatment.

What We Offer

Screening is the starting point. We bring together prevention, understanding, navigation, referral, and follow-through.

  • Preventive Health Screenings

We offer these screenings at the Center, community events, and partner locations:

    • Diabetes screening — blood sugar checks
    • Hypertension screening — blood pressure checks
    • Oral health screening — dental and gum health checks
    • Vision screening — basic eyesight checks
    • HIV screening — HIV testing and connection to care
    • Mental health screening — emotional well-being assessments and referrals
    • Maternal health screening — identifying pregnancy and postpartum health concerns, with referrals and support through ProMoCare
  • Health Education

Plain-language information, in your language, about:

    • How to prevent common health problems
    • What your screening results mean
    • Questions to bring to your doctor
  • Community Health Worker Navigation

    Culturally responsive help with:

    • Next steps after your screening
    • Filling out forms
    • Making appointments
    • Transportation
    • Understanding the healthcare system
  • Referral & Follow-Up Support

    • Warm referrals to healthcare providers
    • Connections to community resources
    • Support to follow through on your next steps
  • Maternal & Child Health

    • Support for mothers during pregnancy and after birth
    • Support for families and children
    • Connection to ProMoCare, the AIFS maternal health program

Services vary by event, staffing, and healthcare partner participation. A screening does not provide a diagnosis or prescribe treatment.

Our team explains your results in clear language, in your language, so you can ask questions.

If you need more care, we explain your referral options and next steps.

4

Follow Through

Community Health Workers help remove barriers and support your next connection.

Our Model

The AIFS RELAIS Pathway

RELAIS guides how we move from reaching people to helping them stay connected to care. It is a navigation framework, not a clinical diagnosis protocol.

R

Reach

Bring prevention information and screening to families at the Center, community events, and partner locations across the Omaha metro.

E

Evaluate

Listen to each person’s questions and understand their screening or navigation need.

L

Look for Risk

Notice screening results or barriers that may need attention, and explain what is known.

A

Advise

Share practical, understandable information and help identify next steps.

I

Integrate

Connect the person to appropriate care or community resources with a warm handoff.

S

Sustain

Support continuity and follow-through when the program includes follow-up.

Screening scope, referral destinations, and follow-up are confirmed by program, event, staffing, consent, and partner participation. Learn more about RELAIS →

Frequently Asked Questions

Screening FAQ

Is the screening free?
Yes. AIFS offers free community preventive health screenings and health education. Specialized services or services from outside healthcare partners may have different eligibility or costs. Our staff will explain any potential costs before making a referral.
Do I need insurance?
No. You do not need health insurance to take part in AIFS community health screenings and health education. If more care is recommended, we can help you understand referral options and connect you with healthcare resources.
What languages are available?
Language help may be available in English, French, Arabic, Ewe, Lingala, Swahili, Nuer, Dioula/Bambara, Fon, Moore, Hausa, and Dinka, depending on staff and interpreter availability. Please contact us before your visit if you need a specific language.
How long does it take?
A basic screening takes about 15–30 minutes. Visits with several screenings, health education, or referral help may take longer.
What happens after the screening?
Our staff review your results with you, provide health education, and explain next steps. If you need follow-up care, our Community Health Workers can help connect you with healthcare providers and community resources.
What services do you provide?
We offer preventive health screenings for diabetes (blood sugar), hypertension (blood pressure), oral health, vision, HIV, mental health, and maternal health. We also provide culturally responsive health education, Community Health Worker navigation, referrals and follow-up support, maternal and child health support through ProMoCare, and community wellness programs. Services may vary by event and partner availability.
Are walk-ins accepted?
It depends on the event, staffing, and services offered. Some programs require registration or an appointment. Please contact us before traveling to confirm.
How do I prepare for the screening?
Most basic screenings need little or no preparation. When you schedule, our staff will tell you if you should bring identification, medication information, or insurance information (if you have it), or follow any special instructions.
Does AIFS provide medical diagnosis or treatment?
No. AIFS provides preventive screenings, health education, navigation, and referral support. Community screenings do not replace diagnosis, treatment, or ongoing care from a licensed healthcare provider. When more evaluation may be needed, we help you connect with healthcare providers.
